FOR YOUR SHOW, PLEASE NOTE:

• ALL DANCERS MUST ENTER TO THEIR MUSIC AND END POSED ON STAGE

VIDEO AND PROFESSIONAL PHOTOGRAPHY SERVICES WILL BE PROVIDED.

• VIDEO is $25 in a case, shipped to your home within 30 days after show.
• PHOTOS are $20 for a minimum of 3 jpegs on a CD mailed to home within 30 days after the show. Photographer will pick the best pictures and send a proof sheet of other good shots. You may then order additional jpegs at $5 per shot.

PLEASE BRING AN ENVELOPE WITH A CHECK OR CASH AND GIVE TO PHOTOGRAPHER FOR PHOTOS AND VIDEOGRAPHY FOR VIDEO (separately) WHEN YOU ARRIVE.

Receipt of this order form by email to MEDLS is your official registration for performance at the indicated MEDLS event and for video and photos AND authorizes the following:

• MEDLS and their staff to provide either Videography or Photography as ordered.
• I agree that I will not hold MEDLS responsible for failure of technical equipment and as such will receive a refund within 30 days if such events should occur.
• I authorize MEDLS to use photos for promotion of other MEDLS events, on the website or related promotion and marketing.

If this form is not received within 7 days or your request to perform,  your slot will be given to a standby dancer, so let us know if there is a problem.
